I was wondering, yesterday as I was gently looking for a rogue to
attack me in STV (which happened rather soon, I make a good bait), I
reduced him down to maybe 15-20% and forced him to run away. I know
vanish was very far from foolproof before, but now it seemed that
vanish overwrote damage over time - the pain I inflicted should have
been enough to dispose of the bugger, but somehow he disappeared and
didn't die...
...did they fix vanish and make it unbreakable, or was I simply a
victim of a lag, and my pain didn't connect server-side?
PS: I'm not complaining in any way, it's more of a "know-your-enemy"
question.

Vanish is certainly not fixed - in fact I think it was NERF'ed a patch
or two back since it seems less reliable these days.
And a DOT will certainly still break it - so my guess is that your
assertion is correct, and your pain did not connect.

Dunno, but in similar situations I've been intrigued by the preparedness
variations among opposing players. Some seem to have items far in
excess of a healing potion and a bandage -- perhaps a Lifestone
[http://www.thottbot.com/?i=12047], a healing belt
[http://www.thottbot.com/?i=3694] (easily craftable at mid-levels), some
whipper root tubers, etc. With many non-shared cooldowns, these items
could explain mysterious healing by your foe.
Also keep in mind the Moist Towelettes
[http://www.thottbot.com/?i=10729] which might negate that Curse of
Agony or SW:P that you were certain would've killed that rogue or at
least popped him back out of stealth... I still have a stack of these in
my bags. :)

Nope. A dot will certainly break vanish, which will also break stealth at
the same time. It's possible he popped a shadow protection pot before he
vanished, the 'Greater' version of those things can suck up more than 3k of
shadow damage. I've used them on occasion in PvP, but they are way too
expensive to use regularly.
