I had the same problem as you, and my fix was to simply turn off a
little program called "RefreshLock" I was running to set custom refresh
rates for my monitor. If you have a similar refresh-rate adjustment
program running at the same time, you'll suffer this problem.

Ya ya ya quit yelling. I was on holidays. :-)
I had the same issue when moving from a GF4 to 6600GT and used the
latest drivers. The latest 77.xx series of nVidia drivers are
crap-in-a-can shitastic.
I loaded up the 77.72 drivers and couldn't run CS:S in full screen, only
a 640x480 window. :-\
I installed the 71.89's instead and it's all good now.
Try that.

This is exactly what Valve says at steampowered.com. Roll your drivers back
to the 71.89 to fix the problem.
