According to ATMA's drop calculator, some of the best areas to find that
are early in hell act three. Spider Forest, Flayer Jungle, and Flayer
Dungeon.

I actually had one drop from Meph's armor rack down in Durance 3 once, which
seems to support what Artdent says about early A3 Hell being perhaps the
best place.

I've had one drop in Durance 1 or 2....

That happened, but I wouldn't get my hopes up too much, because that was
only one hit for that particular item in literally thousands of attempts.
Let's just say that getting an HoZ from Meph's armor rack is possible, but
extremely unlikely.

All the HoZ's I've seen drop, now that I think about it, have been between
Trav/Durance and Big D, hell difficulty. Small sample though.
