I believe preorders of WW received the OOT for free before WW
came out.

Not quite a combination of your last two choices: OoT/Master Quest was
offered as a pre-sell premium for those who pre-purchased or reserved a
copy of Wind Waker. On most pre-sell/reservation receipts, OoT/MQ was
priced at $0.01, with the balance of the pre-sale/reservation amount
going towards Wind Waker (this was my personal experience). The OoT/MQ
package was, however, marked "Not for Resale" (meaning that it wasn't to
be sold as new separately from Wind Waker).
